From Spots to Spotless: How PicoSure Clears Unwanted Pigment

What is PicoSure Laser ?

PicoSure is a specific brand of picosecond lasers (PSLs)—a technology that relies on extremely
short laser pulses delivered in order to break down unwanted skin pigments. The name comes
from the SI prefix “pico”, which indicates that PSL energy pulses are delivered on the order of 1 x 10 -12 seconds. PicoSure received FDA clearance in 2012 for tattoo removal and pigmented lesions. Currently, it is used to treat a variety of pigment problems, including sun spots, acne
scars, and tattoo removals.

The Problem:

For PicoSure, patient concerns arise from pigmented species trapped within the skin, whether it
be an excess of melanin (a natural skin pigment responsible for skin and hair tone) or the large

ink particles that form tattoos. Because these species are located deep within the skin—either in
the lower epidermis or the dermis, the removal of pigmented particles without damaging nearby
tissue poses a challenge for conventional nanosecond (Q-switched) lasers. Furthermore,
depending on a patient’s skin tone, the application of heat in such settings introduces the risk of
burning, scarring, or postinflammatory hyperpigmentation. However, PicoSure avoids these
dangers altogether by relying on a mechanism other than heat.

The Mechanism:

PSLs deliver focused laser pulses—tuned to a wavelength that is absorbed by pigment or tattoo
ink—on the order of 1 x 10

-12 seconds, a minuscule duration that still allows for the release of
energy in targeted areas. Because melanosomes take considerably longer to significantly
diffuse heat to surrounding tissue, these laser pulses can deliver energy to pigment particles
faster than heat can spread and damage healthy areas of the skin. In other words, instead of
pigment being photothermally burned and introducing the risk of heat transfer, it is instead
photomechanically shattered by a laser-induced burst of pressure. Pigment fragments are
naturally cleared out by the body throughout the course of recovery.

Benefits and Risks of PicoSure Laser

Compared to other traditional Q-switched lasers, PSLs are much safer, faster, and less invasive.
Aside from clearing out unwanted pigmentation, energy pulses can stimulate cellular renewal
and boost collagen production, resulting in both a smoother skin texture and a reduction in the
appearance of skin imperfections such as blemishes or wrinkles. PSL treatment can also induce
the shedding of damaged scar tissue and thus promote the creation of new, healthy skin.
Although PicoSure’s mechanism limits the possibility of post-treatment complications, some
risks are associated with not following the guidance of the administering medical practitioner
following the procedure. Temporary side effects such as redness and swelling are plausible, and
post-inflammatory hyperpigmentation or hypopigmentation may arise if treated areas are not
adequately cared for.

least a week prior to treatment, while simultaneously keeping the treatment area clean, healthy,
